Patients who no longer wish to permit their health information to be exchanged to support their care may request to revoke providers’ access to their personal health information. To revoke access, please print and complete the Revocation form (English or Spanish).  You may email, fax or mail your completed form for processing.  The form is typically processed within 24 to 72 hours from notification.
