After you deliver, your nurse will give you a Vital Statistics Worksheet to complete. Before you leave the hospital, your nurse will provide a birth verification letter with the details that will be submitted for your baby’s birth record. Please review it carefully before you sign, check the spelling of all names and all birth details. Corrections are easiest to make before the information is sent to Texas Department of State Health Services (DSHS) Vital Statistics.
You will not automatically receive a certified copy of the birth certificate. You will need to order it separately. The fee is typically $23, plus any additional processing or shipping fees. You can order online through Texas DSHS Vital Statistics (using the state’s approved online ordering service), or you can order in person through the county clerk/local registrar in the county where the birth occurred.
Processing times vary based on the state’s current workload.
Your baby’s information will be sent to the Social Security Administration to request a Social Security number only if you authorize it on the Vital Statistics Worksheet. If you choose this option, the Social Security card will be mailed to the address you provide.
When adding your baby to a health insurance plan, many insurance companies will accept the hospital birth verification letter temporarily. Check with your plan for specific requirements and timelines.
