Assessing Community Health
Every three years, Texas Health embarks on a journey to understand its communities' healthcare needs and focus investments and interventions where they are needed most. Through focus groups, stakeholder interviews and data analysis from our comprehensive Community Health Needs Assessment (CHNA), we unearth health and well-being trends and gaps in our 16-county service area.
Our most recent CHNA, completed in 2022, identified the following community health priorities:
- Health awareness, literacy and navigation.
- Chronic disease prevention and management.
- Behavioral health support.
In collaboration with other community health and social service agencies, schools and municipal leaders, faith-based organizations, academic institutions and other stakeholders, Texas Health is deploying practical, community-driven strategies and funding to improve health outcomes and equity and remove barriers to care across our 16-county service area.
These efforts are led and supported by Texas Health’s Community Health Improvement, North Texas Healthy Communities, Community Affairs, Government Affairs and Advocacy, Texas Health Behavioral Health, Faith & Spiritual Integration and the Texas Health Resources Foundation.
