To combat diabetes and heart disease in Collin County adults, Texas Health is taking a new approach to community health improvement. Introducing the Community Resource Center, offering access to condition-specific care, including bloodwork and education, personalized navigation services, and access to nutrition counseling and support groups—all in one location.
This approach is designed to address barriers that are preventing good health, including providing access to fresh produce and shelf-stable items for those who qualify.
To qualify for the approach, adults ages 18-64 must:
- Live in Collin County
- Have a household income at or less than 250 percent of Federal Poverty Level
- Be diagnosed with specific chronic conditions—congestive heart failure, diabetes, high cholesterol and/or high blood pressure
- Be without health insurance or have insurance with a high deductible
