Texas Health Plano adds new healthcare delivery model

ARLINGTON, Texas — Texas Health Care at Home, an innovative model that brings acute-level care directly into patient homes, is now available to qualifying patients within a 25-mile radius to Texas Health Presbyterian Hospital Plano. Care at Home was first launched by Texas Health in 2022.

“We are partnering with consumers to provide a lifetime of health and well-being, and that means thinking beyond traditional healthcare models,” said Kathi Cox, Ambulatory and Virtual Channel chief operating officer. “We are supporting patients wherever they are.”

Texas Health Plano is the fifth hospital in the system to offer this medical care delivery model. This service is open to acute-care patients who meet certain criteria, including having a caretaker who can assist a patient in the home. It is available to older patients with Medicare and Medicare Advantage plans who are diagnosed with diseases such as heart failure, chronic obstructive pulmonary disease (COPD), asthma, pneumonia, cellulitis/soft tissue infections and complicated urinary tract infections.

Patients are cared for at home by a mix of providers who work in unison with healthcare professionals stationed at a virtual medical command center.

Care at Home is also offered at Texas Health Harris Methodist Hospital Alliance, Texas Health Harris Methodist Hospital Fort Worth, Texas Health Harris Methodist Hospital Southwest Fort Worth and Texas Health Harris Methodist Hospital Hurst-Euless-Bedford.

Texas Health continues to integrate advanced technology amid changing consumer demands, with an eye toward emerging health industry trends.

The hospital has served Plano and surrounding cities for more than 30 years. It has grown alongside the city, adding spaces and services to connect consumers to quality medical care. For example, plans are underway for the construction of an eight-story tower on the hospital campus.
