Graduate Medical Education (GME) residents are encouraged to voice concerns, make suggestions for improvement, and provide general feedback on the working and learning environment experienced at a Texas Health facility through the program evaluation tools and/or directly to the GME program director, associate program director, faculty and/or program coordinator. In addition, Texas Health provides an opportunity to report a concern, issue, or suggestion anonymously.

Identifying information is not required to complete the form below. The IP address from which your report is submitted will not be included in your report. Follow-up on a concern, issue or suggestion may only occur if you choose to provide contact information.

Please include as much detail as possible about the concern, issue, or suggestion. The information you provide will be sent to the Texas Health Resources chief academic officer and system director of medical education.
